AI Photoshoot Studio — Participant Consent, Release & Media Authorization

Event: Sewanhaka High School · Provider: Turner Family Portraits (“TFP”)

Please print this form, sign where indicated, and return it to a TFP staff member before participating.

In exchange for the opportunity to participate in the TFP AI Photoshoot Studio (“the Studio”) at the event above, I (“Participant”) agree to the following:

1. Voluntary participation. My participation is voluntary. I am at least 18 years old, or a parent/legal guardian signing for the minor named below.
2. Consent to AI portrait creation. I authorize TFP to use my selfie and likeness to create AI-generated portraits of me using Google’s Gemini image models. I understand these are stylized, computer-generated interpretations, and that results vary and are not guaranteed.
3. Express consent to biometric face-matching. I give my express written consent for TFP to collect and process a biometric identifier derived from my selfie — a facial-geometry template, created using Amazon Rekognition (AWS’s managed face-analysis service) — for the sole purpose of matching me to my own photos from this event. I understand this face data is used only for that purpose and never for advertising, tracking, profiling, or sale; is isolated to this event in its own dedicated face collection and never pooled across events or clients; is not used to train or improve any AI model; is processed in the United States (AWS US East); and is permanently deleted when the event is archived.
4. Release of liability & assumption of risk. To the fullest extent permitted by law, I release, waive, and hold harmless TFP and its owners, employees, contractors, and partners from any and all claims, liabilities, or damages arising out of or relating to my participation, the AI portraits, or the handling of my data — except for gross negligence or willful misconduct.
5. Data-handling acknowledgment. I acknowledge that TFP has informed me how my data is handled, as described on TFP’s Privacy & Security page (aipstudio.tfp.nyc/security). TFP never sells personal data and never uses guest images or face data for advertising. I may request access to or deletion of my data — including my event face data — by emailing privacy@tfp.nyc; my face data is in any case deleted when the event is archived.
6. Entire agreement; severability; governing law. This is the entire agreement between me and TFP regarding the required terms above, and is governed by the laws of the State of New York. If any provision is held unenforceable, the remainder stays in effect.
